fix(limrun): identify instance creation with User-Agent
Summary
Send User-Agent: agent-device-cli/<version> on Limrun iOS and Android instance creation for partner attribution. Remove the unused x-agent-device-client and x-agent-device-version defaults and their options builder; verification no longer needs a client version.
Test creation and cleanup headers through the Limrun SDK, verify the CLI runtime supplies its version, and document attribution. Retire the three stale test-kit eager-import approvals that failed Coverage: those entries already exist at the merge-base, where the no-growth rule owns them.
Scope: 11 files; Limrun provider and caller cleanup, tests/docs, and the requested CI gate repair.

pnpm check:affected --run: all runnable checks passed on this exact head (full local check set).
Integration Tests and Linux CI passed on this head; remaining CI is pending. The previous iOS XCTest startup timed out and is awaiting its new run. Limrun-side attribution has not been live-verified.
